As you will be participating in the HeppCare programme, you must read, acknowledge, and consent to the following.

1. Data Processing and Image Use

In accordance with Article 4(11) of the General Data Protection Regulation (GDPR) and UK Data Protection Act 2018, we require your consent to hold and process your personal data at Hepp Ltd. We keep several categories of personal data on our HeppCare participants to conduct research on and publish findings on the correlation between mental and physical wellness and engagement with sports and artistic activities. This data will be stored in Hepp Ltd’s computer systems.

Specifically, we wish to hold the following types of data:

• Personal details: name, age, ethnic group, financial

• Information of any mental health issues you have experienced or other medical information

• Information on your gender, caring responsibilities, disability, age range, ethnic origin and how you heard about HeppCare for equality monitoring purposes

• Information gathered prior to starting the HeppCare programme

By signing this form, you are consenting to Hepp Ltd collecting, holding and processing your personal data to be used in future research publications. Participants’ identities will be anonymised in all publications.

Images of participants from HeppCare sessions may be used in Hepp’s promotional media and publications.

2. Health and Safety Precautions

You must ensure that you take appropriate care of your own health and safety during HeppCare sessions. Hepp Ltd will not be liable for participants’ accidental or own negligent injuries. Hepp Ltd remains bound to Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013. This clause does not affect your statutory rights.

Pregnant people are not allowed to take part in the HeppCare programme. You must not attend HeppCare sessions after consuming alcohol or taking drugs. Participants are only allowed in training spaces when accompanied by a member of staff.

I confirm that I have read and understood the above.

If, at any time, you wish to withdraw or change your consent to Hepp Ltd processing your personal data, you may do this at any time by contacting Hepp Ltd, 86-90, Floor 3, Paul Street, London, EC2A 4NE, United Kingdom or info@hepp.uk. Please note that all processing of your personal data will cease once you have withdrawn consent, other than where this is required by law, but this will not affect any personal data that has already been processed prior to this point.
